By participating in the activities and programs of the Surprise Tennis Club, I agree to the following:
Sportsmanship & Conduct
I agree to uphold the principles of good sportsmanship at all times during my involvement with the Surprise Tennis Club. This includes treating all players, officials, captains, and organizers with respect and courtesy. I understand that failure to adhere to these standards, or any violations of the Club’s mission, values, or behavioral expectations, may result in disciplinary action, including removal from the team or club activities and restrictions on future participation.
Fees & Payment Obligations
I agree to pay all applicable fees associated with participation, including but not limited to:
A $44 USTA annual membership fee, payable at the time of registration.
Team registration fees ranging from $20 to $40, due at registration.
Team court fees estimated between $30 and $75, which vary based on court costs at STRC, the number of home matches, and the number of participants.
A $10 Surprise Tennis Club membership fee.
I understand that all court and membership fees must be paid to the Surprise Tennis Club by the specified deadline. Failure to make timely payments may result in removal from the team or ineligibility to participate until all fees are paid in full.
Media Release
I grant the Surprise Tennis Club permission to capture and use photographs and videos of me during club events and activities for promotional and educational purposes. These materials may be used across various platforms, including social media, websites, and other marketing channels. I acknowledge that my image and likeness may be used in perpetuity without compensation. I understand that I may withdraw this consent at any time by submitting a written request; however, content already published prior to withdrawal may continue to be used.
Consent to Share Contact Information
I consent to the sharing of my contact information with the Surprise Tennis Club and team captains for the purpose of official club communications, such as meeting notices, event updates, and other relevant club activities. I understand that I may opt out of club-wide email or text communications at any time. I acknowledge that no mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

By signing or otherwise accepting this agreement, I affirm that I have read, understand, and agree to abide by all
Surprise Tennis Club Terms & Policies.
